Cefpodoxime: Uses, Dosage, Side Effects & Interactions
Table of Contents
Introduction to Cefpodoxime
Cefpodoxime is a third-generation cephalosporin antibiotic used to treat various bacterial infections. As an oral medication available in tablet and suspension forms, cefpodoxime is prescribed for mild to moderate infections caused by susceptible strains of Gram-positive and Gram-negative bacteria. Its broad spectrum of activity makes it effective against many common infectious agents.
Therapeutic Uses
Cefpodoxime is indicated for the treatment of the following bacterial infections:
- Respiratory tract infections (e.g., bronchitis, pneumonia, sinusitis, pharyngitis)
- Urinary tract infections
- Skin and soft tissue infections
- Otitis media (middle ear infection)
The antibiotic is effective against susceptible strains of bacteria, including Streptococcus pneumoniae, Haemophilus influenzae, and Moraxella catarrhalis.
Dosage Forms and Strengths
Cefpodoxime is available in two oral dosage forms:
- Tablets: 100 mg and 200 mg
- Oral suspension: 50 mg/5 mL and 100 mg/5 mL (when reconstituted)
The recommended dosage varies depending on the type and severity of the infection, as well as the patient’s age and renal function. Typically, adults are prescribed 100-400 mg every 12 hours, while children receive weight-based doses ranging from 5-10 mg/kg/day in two divided doses.
Administration
Cefpodoxime tablets should be taken orally with or without food. The oral suspension must be reconstituted before use and should be administered with a measuring device to ensure accurate dosing. Patients should complete the entire course of therapy, even if symptoms improve, to prevent bacterial resistance and relapse of the infection.
Side Effects
Common side effects of cefpodoxime include:
- Gastrointestinal issues (e.g., diarrhea, nausea, vomiting)
- Headache
- Dizziness
- Rash
Rare but serious side effects may include severe allergic reactions, Clostridium difficile-associated diarrhea, and liver dysfunction. Patients should seek medical attention if they experience severe or persistent adverse reactions.
Warnings and Precautions
Cefpodoxime should be used with caution in patients with a history of allergies to cephalosporins or penicillins, as cross-reactivity may occur. The antibiotic should also be used cautiously in patients with renal impairment, as dosage adjustments may be necessary. Prolonged use of cefpodoxime may result in the overgrowth of nonsusceptible organisms, including fungi.
Interactions
Cefpodoxime may interact with certain medications, including:
- Antacids containing magnesium or aluminum
- H2 receptor antagonists (e.g., famotidine, ranitidine)
- Probenecid
These interactions can affect the absorption or excretion of cefpodoxime, potentially reducing its effectiveness or increasing the risk of side effects. Patients should inform their healthcare provider about all medications they are taking before starting cefpodoxime therapy.
Contraindications
Cefpodoxime is contraindicated in patients with a known hypersensitivity to cefpodoxime or any other cephalosporin antibiotics. It should also be avoided in patients with a history of severe allergic reactions to penicillins, as cross-reactivity between beta-lactam antibiotics may occur.
